ACCME Objectives
  1. Collaborate across clinical specialties to provide individualized diabetic care
  2. Determine the appropriate diabetic management options based on their patient's clinical presentation
  3. Recognize the challenges T1D patients encounter in transitioning from pediatrician to adult medical provider
  4. Understand a patients journey through a cardiometabolic approach to treatment.

ACPE Pharmacist Objectives
  1. Discuss the clinical standards and guideline updates for treating Type 1 Diabetes.
  2. Outline which conditions fall under the cardiometabolic umbrella.
  3. Review treatment approaches to weight management in patients with diabetes.
  4. Review how to evaluate & interpret a continuous glucose monitor report.
  5. Apply continuous glucose monitor assessment concepts to a specific patient case.
  6. Review documentation requirements associated with continuous glucose monitor billing.
  7. Demonstrate the importance of interdisciplinary collaboration to support therapy management for patients with diabetes.
