Reducing harm from anticoagulant therapies is a priority to multiple national professional and governmental agencies. Recent literature and guidelines should mediate important shifts in conventional approaches to perioperative bridging of anticoagulant therapies, yet uptake of these data and recommendations into practice remains challenging. This active learning session will feature a case-based format whereby 4 patient cases will be sequentially presented, worked through in mini break-outs, and then explained through connection to primary literature and clinical practice guidelines. It is designed to be a very timely and high-yield program for practicing inpatient clinicians.


Objectives
After completion of this activity, participants will be able to:

  1. Discuss current strategies for perioperative antithrombotic management
  2. Develop anticoagulant treatment plans for complex inpatient scenarios
  3. Compare bleeding and thrombotic event rates between available anticoagulants
