Adult hemiplegia is one of the leading causes of disability in the United States today.  For occupational therapists, being proficient in the assessment of treatment of the upper extremity hemiplegia is vital while treating clients in the neurological specialty.  This course in reviewing the upper extremity from the proximal perspective.  Participants will learn about the complex force couples that are necessary for healthy shoulder motion and the common deviations after stroke.  Additional content will review evidence-based methods for assessing the scapula, causes of scapula dyskinesia and treatment approaches to strengthen desired force couples at the scapula. Overcoming the weight of the weakened arm with hemiplegia is an extremely common challenge for both the stroke survivor and the therapist charged with helping the client achieve maximal recovery.  This topic will be heavily reviewed in both lecture and lab formats through a hierarchical model of closed chain exercises/functions that can be individualized for each client’s unique needs.     


Objectives
After completion of this activity, participants will be able to:

  1. Identify levels of impairment related to the Brunnstrom’s level of recovery scale and the Fugl Meyer Upper Extremity Assessment and understand the movement characteristics related to these scales.
  2. Identify levels of impairment related to the Brunnstrom's level of recovery scale and the Fugl Meyer UE assessment and understand the movement characteristics related to these scales!
  3. Identify the anatomy / muscles / motions needed to facilitate healthy force couples of the shoulder girdle, scapula, and trunk to promote selective motion and neurological advancement of the upper extremity.
  4. Understand the phases of reach and neurological mechanisms that drive and advance this movement skill.
  5. Identify the anatomy/muscles/motions needed to facilitate healthy force couples of the shoulder girdle, scapula, and trunk to promote selective motion and neurological advancement of the UE.
  6. Understand methods to performing clinical assessments of the scapula and shoulder girdle.
  7. Understand the phases of reach and neurological mechanisms that drive and advance this movement skill.
  8. Understand scapular dyskinesis- what are the causes and how to identify in static postures and dynamic motion.
  9. Understand methods to performing clinical assessments of the scapula and shoulder girdle.
  10. Understand how the trunk and lower body base of support affects and interacts with upper extremity movement and function.
  11. Understand scapular dyskinesis - what are the causes and how to identify in static postures and dynamic motion.
  12. Understand the effect of posture has on healthy shoulder motions.
  13. Understand how the trunk and lower body base of support affects and interacts with upper extremity movement and function.
  14. Understand a scapular rehabilitation algorithm that can assist with clinical assessment and treatment strategy prioritization. This includes the influence of soft tissue flexibility, muscle performance and motor control.

  16. Understand how movements performed in closed chain set ups positively influence the motor sequence of forward reach and healthy force couples.
