Completion of the HCC Module is geared to enhance the documentation process for managing and updating medical condition diagnoses. This is a recording that provides the elements to document accurate diagnosis code assignments incorporating a patient centered practice workflow to proactively assure patient diagnoses are maintained and updated in the medical record.

After completion of this activity, participants will be able to:

  1. Describe how HCC coding can improve patient care.
  2. Explain how HCC coding can improve your practice's financial bottom line.
